Terms of Affiliation
Required documents and adopted decisions for the registration of a doctor in the Order of Physicians
- First: The physician shall come in person to the Order to submit the membership application and to make an interview with the administrative committee.
- Second: Submit the original copy with legalized copies hereof of all the required certificates and documents to be read and verified by the administration and the administrative committee in LPO, and submit the following documents:
- 2 passport photos
- Original Police Record not exceeding three months
- Original Individual extract not exceeding six months
- Certified copy of the Lebanese baccalaureate diploma – second part (the original copy to be presented)
- In case a Lebanese baccalaureate diploma is not available, a copy of the certified equivalent certificate shall be submitted (the original copy to be presented) in addition to a certified copy of the equivalence decision by the Ministry of Education (the original copy to be presented)
- A certified copy of the Medicine Certificate and number of the years of study and the training year (the original copies to be presented)
- A certified copy of the Medicine Certificate Equivalence (the original copy to be presented)
- A certified copy of the Colloquium Attestation (the original copy to be presented)
- A certified copy of the license to practice the medical profession issued by the Ministry of Health (the original copy to be presented)
- Certified copies of the certificate or certificates of specialization with detailed number of specialization years (the original copy to be presented)
- A certified copy of the license to hold a medical specialization title issued by the Ministry of Health (the original copy to be presented)

* In case of multi specializations, an amount of 3,255,000 LBP shall be added to the annual fee for each specialization.
For physicians graduated from institutes outside Lebanon:
Detailed number of years of study and the training year in addition to an attestation from the competent authorities in the country granting the certificate duly certified and proving that the medical certificate including the training year authorizes its holder, in case meeting the citizenship term, to practice the medical profession in said country without any limit or condition – a copy shall be submitted (an original copy to be presented).
